ABSTRACT

Resumen Objetivo: explorar las reacciones de contratransferencia de los terapeutas en la fase inicial de la psicoterapia psicoanalítica (entre las sesiones 4 y 5) y verificar las posibles asociaciones entre estas y las características personales del paciente (sociodemográficas, psicológicas y psicopatológicas). Método: estudio naturalista, transversal, correlacional y observacional con enfoque descriptivo-analítico. La muestra consta de 180 pares de pacientes y terapeutas, de los cuales 180 eran pacientes adultos (32.82 ± 12.66 años) y 48 terapeutas (32.69 ± 10.01 años). Resultados: la contratransferencia se ve afectada por la edad del paciente, los aspectos psicológicos de su vida anterior (traumas y apego parental) y elementos de su personalidad (habilidades de empatía y mecanismos defensivos), pero no por los síntomas que presenta. Conclusiones: los hallazgos respaldan la literatura científica, que señala la relevancia de las experiencias infantiles y las características de personalidad del paciente para evocar patrones específicos de contratransferencia en el terapeuta. Conocer tales relaciones puede ayudar a los clínicos a identificar sus propias reacciones y usarlas terapéuticamente, según corresponda.


Resumo Objetivo: explorar as reações contratransferenciais de terapeutas na fase inicial de psicoterapia psicanalítica (entre a 4a e a 5a sessão) e verificar possíveis associações destas com as características pessoais do paciente (sociodemográficas, psicológicas e psicopatológicas). Método: estudo naturalístico, transversal, correlacional e observacional de abordagem descritivo-analítica. A amostra é composta por 180 duplas paciente-terapeuta, dos quais 180 eram pacientes adultos (32,82 ± 12,66 anos de idade) e 48 terapeutas (32,69 ± 10,01 anos de idade). Resultados: a contratransferência é afetada pela idade do paciente, aspectos psicológicos da sua vida pregressa (traumas e vinculação parental) e elementos de sua personalidade (habilidades empáticas e mecanismos defensivos), mas não pelos sintomas. Conclusões: os achados apoiam a literatura científica, a qual sinaliza a relevância das experiências infantis e de características da personalidade do paciente na evocação de padrões contratransferenciais específicos no terapeuta. Conhecer tais relações pode ajudar os clínicos a identificar suas próprias reações emocionais e utilizá-las terapeuticamente, quando for o caso.


Abstract Objective: to explore therapists' countertransference reactions in the initial phase of psychoanalytic psychotherapy (between sessions 4 and 5) and to verify possible associations between these and the patient's personal characteristics (sociodemographic, psychological, and psychopathological). The sample consists of 180 patient-therapist dyads, of which 180 were adult patients (32.82 ± 12.66 years old) and 48 therapists (32.69 ± 10.01 years old). Results: countertransference is affected by the patient's age, psychological aspects of his or her previous life (traumas and parental attachment) and elements of personality (empathetic skills and defensive mechanisms), but not by the showed symptoms. Conclusions: the findings support the scientific literature, which points to the relevance of childhood experiences and personality characteristics of the patient to evoke specific countertransference patterns in the therapist. Understanding such relationships can help clinicians identify their own reactions and deliver the therapy, as appropriate.

@#Background and objectives: The cumulative time spent without medical intervention in acute stroke patients may affect clinical outcomes. As the onset-to-arrival time to the hospital is crucial for effective treatment interventions, this study aimed to explore the factors associated with pre-hospital delays amongst acute stroke patients. Methods: We explored 932 patients data retrieved from the National Neurology Registry of Seberang Jaya Hospital between January 2013 and December 2018. Data on patient demographics and stroke manifestations were analysed using descriptive, univariate and multivariate logistic regressions. Results: Most patients were men (62.9%) with an average age of 62 years old. In the final multivariate regression model, pre-hospital delay was significantly lower among Chinese patients (aOR=0.6, 95% CI 0.4–0.9, p=0.016) and those using hospital ambulance (aOR=0.4, 95% CI 0.3–0.7, p<0.001), but higher among patients with lacunar infarcts (aOR=2.5, 95% CI 1.4–3.3; p<0.001). Conclusions: Demographic characteristic (ethnicity) and stroke manifestations, particularly stroke subtypes, and mode of transport were mainly associated with pre-hospital delays among acute stroke patients.

ABSTRACT

Introduction: We started the gender-specific clinic for women to provide sufficient treatment for female patients. The purpose of this study was to clarify the characteristics of patients using the gender-specific clinic for women, and to assess the association among depression, physical and mental subjective symptoms.Methods: This observational study included female patients aged 16-84 years who visited our clinic between June 2012 and December 2015 (N=97). In addition to general attributes, we collected data on physical and mental symptoms, and depression status using the Cornell Medical Index (CMI) and Self-rating Depression Scale (SDS), respectively, at the first visit. We conducted analyses to assess patient characteristics and the association between subjective symptoms and depression, and between physical and mental symptoms by estimating odds ratios (ORs) and 95% confidence intervals (CIs).Results: The average age of subjects was 50.4 years. The average CMI score was 42.7 points and 55.9% of the subjects were suggested to be neurotic. The average SDS score was 45.0 points and 64.0% of them were suggested to be depressed. The association with depression by SDS was observed in subjective symptoms of CMI such as fatigue (OR [95%CI]: 7.66 [2.26-25.99], p-value: 0.001) and anxiety (OR [95%CI]: 11.73 [1.80-∞], p-value: 0.006). Physical symptoms in the cardiovascular system were positively association with some mental symptoms such as tension.Conclusion: As female patients often have mental symptoms, it is essential for doctors engaging in gender-specific medicine for women to approach patients while considering psychological and mental aspects.

@#BACKGROUND: Fluid and electrolyte balance is a key concept to understand for maintaining homeostasis, and for a successful treatment of many metabolic disorders. There are various regulating mechanisms for the equilibrium of electrolytes in organisms. Disorders of these mechanisms result in electrolyte imbalances that may be life-threatening clinical conditions. In this study we defined the electrolyte imbalance characteristics of patients admitted to our emergency department. METHODS: This study was conducted in the Emergency Department (ED) of Uludag University Faculty of Medicine, and included 996 patients over 18 years of age. All patients had electrolyte imbalance, with various etiologies other than traumatic origin. Demographic and clinical parameters were collected after obtaining informed consent from the patients. The ethical committee of the university approved this study. RESULTS: The mean age of the patients was 59.28±16.79, and 55% of the patients were male. The common symptoms of the patients were dyspnea (14.7%), fever (13.7%), and systemic deterioration (11.9%); but the most and least frequent electrolyte imbalances were hyponatremia and hypermagnesemia, respectively. Most frequent findings in physical examination were confusion (14%), edema (10%) and rales (9%); and most frequent pathological findings in ECG were tachycardia in 24%, and atrial fibrillation in 7% of the patients. Most frequent comorbidity was malignancy (39%). Most frequent diagnoses in the patients were sepsis (11%), pneumonia (9%), and acute renal failure (7%). CONCLUSIONS: Electrolyte imbalances are of particular importance in the treatment of ED patients. Therefore, ED physicians must be acknowledged of their fluid-electrolyte balance dynamics and general characteristics.

ABSTRACT

Objective This article was to focus on the study of patient's profile,type and distribution of the disease and the related factors contributing to medical cost on 14398 cases of hospitalized patients with hepatitis B from a hospital of infectious diseases located in Beijing,in order to provide basic information on optimizing the prevention and treatment strategies.Methods Information on hospital admission of patients、with hepatitis B was collected and SPSS 16.0 statistics software package was used to analyze the profile,disease patterns distribution,structure of medical costs and main contributors related to medical costs.Results Through analysis,we found that the proportion of male patients was much bigger than that of female patients,with ratio as 2.9.The average age of patients with hepatitis B was 45.2 years old.The treatment process was time consuming,and the mortality rate was hiigh.Our data showed that the mortality of hospitalized patients was up to 7%and the average age of death was 55 years old.Hepatitis B infection was easy to develop into chronic,cirrhosis and even liver cancer,Fortreatment cost,the largest cost share was the drugs being used which accounted for 62.4%.In terms of health care costs,it was high and the total cost of hospitalization was related to the following factors:days of hospitalization;complexity of the disease condition and the factors as the severity of the disease complications as surgery,frequencies of rescue,type of disease etc;basic information of the patients as age,occupation,origin.Conclusion Compared with other diseases,the proportion of male patients with hepatitis B was much higher than that of the famale.Hepatitis B had a longer duration and difficult,treatment high cost of medical care,poor prognosis,high mortality rate;Cost control,Call not simply fixcd by single disease,but by complexity of the disease and patient characteristics.

ABSTRACT

BACKGROUND: Bronchoscopy is an important diagnostic and a therapeutic tool in chest medicine. However, most patients feel that a bronchoscopy is an unpleasant procedure, and it is important to sedate the patients appropriately, particularly where repetitive examinations are required. Midazolam is a sedative drug with amnestic qualities and a rapid 2 hour half-life. This study have attempted to determine the safety, appropriate dosage, and the effect of midazolam premedication in patients who underwent a bronchoscopy. METHODS: One hundred and eighty consecutive patients undergoing bronchoscopy were enrolled in this study. The patients received a midzolam doses of 0.03 mg/kg, 0.06 mg/kg, or a placebo. An additional dose of lidocaine, the total number of coughs, and the duration of the procedures were recorded with monitoring the the blood pressure, heart rate, and oxygen saturation. The level of satisfaction was assessed by the patient, bronchoscopist, and the nurse. RESULTS: The blood pressure, pulse rates, oxygen saturation, number of coughs, lidocaine dose, and procedure time in the 3 groups were similar. There was a trend for the midazolam 0.03 mg/kg group to satisfy bronchoscopists more than the other two groups. The nurses' acceptability was lower in the midazolam 0.06 mg/kg group than the other groups. The patients' acceptablity was greater in both the midazolam 0.03 mg/kg and 0.06 mg/kg groups than in the control group. CONCLUSION: Sedation with low doses of intravenous midazolam is a safe technique for fiberoptic bronchoscopy with a low morbidity and high acceptable to patients and bronchoscopists.
